Terms & Conditions

Introduction
These Terms & Conditions (“Terms”) govern your use of the Blue Sky IV PLLC website, your purchase of services, appointments, memberships, mobile wellness services, communications, and related interactions with Blue Sky IV PLLC (“Blue Sky IV PLLC” “Company,” “we,” “us,” or “our”). By using our website, submitting an appointment request, scheduling an appointment, purchasing services, purchasing a membership, or receiving services from Blue Sky IV PLLC you acknowledge that you have read and agree to these Terms. If you do not agree with these Terms, do not use the website or purchase services.
1. About Blue Sky IV PLLC
Blue Sky IV PLLC provides mobile IV hydration and wellness-related services through appropriately licensed healthcare professionals, subject to applicable Illinois law, professional scope of practice, clinical protocols, medical screening, and provider judgment. Services may include, depending upon availability and applicable law: • IV hydration; • Vitamin or nutrient infusions; • Vitamin injections; • Wellness assessments; • Certain laboratory services; • Other wellness services; • Other services specifically identified by Blue Sky IV PLLC. Not every service is available to every person. Blue Sky IV PLLC may modify, suspend, discontinue, or limit services at any time.
2. Clinical Services are Subject to Professional Judgment
The purchase or scheduling of a service does not guarantee that the service will ultimately be provided. Before providing a clinical service, a qualified healthcare professional may: • Review medical history; • Review medications; • Review allergies; • Obtain vital signs; • Conduct a screening assessment; • Determine whether the requested service is appropriate; • Modify the requested service; • Decline to provide the requested service; • Recommend that the individual seek another level of care. Clinical decisions will be based on applicable professional standards, clinical protocols, scope of practice, and the patient’s circumstances.
3. No Guarantee of Treatment Eligibility
A person may be determined to be ineligible for a requested service because of: • Medical history; • Current symptoms; • Medication use; • Allergies; • Pregnancy or suspected pregnancy; • Vital signs; • Clinical findings; • Dehydration severity; • Potential infection; • Cardiovascular concerns; • Renal concerns; • Other contraindications; • Insufficient information; • Provider judgment; • Safety concerns. Blue Sky IV PLLC may refuse or discontinue a service when a provider determines that proceeding would not be appropriate.
4. Wellness Services are not Emergency Care
Blue Sky IV PLLC is not a substitute for: • A hospital emergency department; • Emergency medical services; • 911; • Urgent emergency evaluation; • Primary care; • Specialist care. Blue Sky IV PLLC does not guarantee immediate availability. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Examples of symptoms requiring immediate emergency evaluation may include: • Severe chest pain; • Severe difficulty breathing; • Loss of consciousness; • Stroke symptoms; • Severe allergic reaction; • Uncontrolled bleeding; • Severe confusion; • Seizure; • Severe or rapidly worsening symptoms.
5. Medical Screening
Before treatment, Blue Sky IV PLLC may require completion of an intake form, medical questionnaire, consent documentation, or other screening. You agree to provide information that is accurate and complete to the best of your knowledge. You must notify the healthcare professional of relevant: • Medical conditions; • Allergies; • Medications; • Supplements; • Prior reactions; • Pregnancy or possible pregnancy; • Recent procedures; • Recent hospitalizations; • Other clinically relevant information. Failure to provide accurate information may affect whether services can safely be provided.
6. Provider Authority
The healthcare professional providing services retains professional authority over clinical decisions. No customer, family member, employer, facility, or other person may require a healthcare professional to provide a service that the professional determines is clinically inappropriate or outside the professional’s lawful scope of practice.
7. Mobile Service Location Requirements
Because Blue Sky IV PLLC provides mobile services, the customer is responsible for providing a reasonably safe and appropriate service environment. The location must: • Be reasonably accessible; • Have sufficient space; • Permit safe placement of equipment; • Permit the provider to perform the service safely; • Have appropriate lighting; • Be free of significant hazards; • Permit appropriate infection-control practices. Blue Sky IV PLLC may decline service if the environment is unsafe. Examples include: • Aggressive animals; • Unsafe conditions; • Smoke exposure; • Illegal activity; • Excessive crowding; • Unsanitary conditions; • Threatening behavior; • Dangerous weather or travel conditions; • Other conditions that may endanger personnel or patients.
8. Appointment Times
Appointment times are estimates and may be affected by: • Traffic; • Weather; • Prior appointments; • Clinical circumstances; • Emergencies; • Provider availability; • Travel conditions. Blue Sky IV PLLC will make reasonable efforts to arrive within the scheduled appointment window.
9. Customer Responsibility for Location Information
Customers must provide accurate: • Address; • Apartment/unit number; • Gate/access information; • Parking information; • Contact telephone number; • Special access instructions. If the provider cannot access the location because the customer provided incomplete or inaccurate information, the appointment may be treated as a missed appointment.
10. Cancellations and Rescheduling
Appointments may be cancelled or rescheduled without a cancellation fee if notice is provided at least 24 hours before the scheduled appointment. Cancellations made less than 24 hours before the appointment may be subject to a $25.00 fee. If you do not cancel and are unavailable when your nurse arrives, your appointment will be considered a no-show and a $100 fee will be charged to the card on file. Our nurses travel specifically to your location and reserve appointment time exclusively for you, so these fees help us fairly compensate our clinical team for reserved travel and appointment time. Emergencies will be reviewed individually at Blue Sky IV's discretion. Blue Sky IV PLLC may waive a fee in its discretion for circumstances such as: • Documented emergency; • Hospitalization; • Severe weather; • Provider cancellation; • Other circumstances determined appropriate by Blue Sky IV PLLC
11. Provider-Cancelled Appointments
Blue Sky IV PLLC may cancel or reschedule an appointment because of: • Provider illness; • Provider emergency; • Weather; • Unsafe travel conditions; • Equipment problems; • Medication or supply availability; • Clinical concerns; • Operational circumstances. If Blue Sky IV PLLC cancels an appointment before services are provided, the customer will not be charged for the cancelled clinical service.
12. Payment
Payment is due according to the pricing displayed at the time of purchase or appointment booking. Blue Sky IV PLLC may require: • Credit card; • Debit card; • Electronic payment; • Other approved payment method. Customers authorize Blue Sky IV PLLC or its designated payment processor to charge the selected payment method for authorized services, fees, memberships, cancellation charges, and other amounts disclosed at checkout.
13. Prices
Prices may change. The price applicable to an appointment will generally be the price disclosed at the time of purchase or booking, unless otherwise stated. Promotional pricing may be subject to separate terms. Discounts cannot be combined unless expressly authorized.
14. Memberships and Recurring Payments
If Blue Sky IV PLLC offers memberships or recurring services, the membership agreement presented at enrollment will govern the specific membership. Before enrollment, the customer will be provided with applicable information concerning: • Membership price; • Billing frequency; • Benefits; • Included services; • Excluded services; • Minimum commitment, if any; • Renewal terms; • Cancellation requirements; • Expiration of unused benefits; • Applicable restrictions. Where a membership automatically renews, Blue Sky IV PLLC will provide the disclosures and cancellation mechanisms required by applicable Illinois law. Illinois’ Automatic Contract Renewal Act requires clear and conspicuous disclosure of renewal terms, recurring charges, term length, and cancellation procedures, and provides requirements for online cancellation.
15. Membership Services are not Medical Guarantees
Membership does not guarantee: • Eligibility for every service; • A particular treatment; • A particular medication or ingredient; • A specific clinical outcome; • Provider availability at every requested time; • Clinical approval of every requested service. Clinical eligibility is determined separately for each service.
16. Refunds
Except where required by law, refunds are governed by the refund policy presented at the time of purchase. Once a clinical service has been provided, Blue Sky IV PLLC generally does not provide refunds based solely upon dissatisfaction with an expected wellness outcome. If a service was not provided because Blue Sky IV PLLC determined it was clinically inappropriate, Blue Sky IV PLLC may refund the applicable unused service charge according to its refund policy. Nothing in this section limits rights provided by applicable law.
17. Promotions
Promotions may have: • Expiration dates; • Eligibility requirements; • Geographic restrictions; • Service restrictions; • Redemption limits; • Membership restrictions; • One-per-person limitations. Blue Sky IV PLLC reserves the right to correct typographical errors and discontinue promotions when permitted by law.
18. Gift Cards
If Blue Sky IV PLLC offers gift cards, additional gift-card terms will apply. Gift cards may not be redeemable for cash except where required by law. Lost or stolen gift cards may not be replaced unless otherwise required by law.
19. Clinical Risks
IV therapy and injections are medical procedures and may involve risks. Potential risks may include: • Pain; • Bruising; • Bleeding; • Swelling; • Infection; • Phlebitis; • Infiltration; • Extravasation; • Dizziness; • Fainting; • Nausea; • Headache; • Fluid-related complications; • Allergic or hypersensitivity reactions; • Medication or nutrient-specific reactions; • Electrolyte disturbances; • Infection; • Other unforeseen complications. The healthcare professional will discuss relevant risks and obtain any required consent before providing the service.
20. No Guarantee of Results
Blue Sky IV PLLC does not guarantee that IV hydration, vitamins, nutrients, injections, wellness services, or other services will produce a particular result. Statements concerning hydration, wellness, energy, recovery, nutrition, or general well-being are not guarantees of individual outcomes. Individual results vary.
21. Supplements, Vitamins, and Nutrients
Ingredients used in a service will be determined according to applicable clinical protocols, provider assessment, availability, and applicable law. Customers should inform the provider of all medications, supplements, allergies, and relevant medical conditions.
22. Medications and Compounded Products
If Blue Sky IV PLLC offers any prescription medication, compounded preparation, or medication-related service, that service may be subject to additional: • Prescriber requirements; • Pharmacy requirements; • State and federal law; • Eligibility criteria; • Clinical protocols; • Consent requirements. Nothing on the website creates a guarantee that a prescription or medication will be provided.
23. Laboratory Services
If Blue Sky IV PLLC offers laboratory testing: • Tests may have limitations; • Results may require clinical interpretation; • Results may not diagnose every condition; • Abnormal results may require follow-up; • Laboratory errors are possible; • Customers remain responsible for appropriate follow-up with their healthcare provider. Blue Sky IV PLLC does not guarantee that a laboratory test will identify every medical condition.
24. Customer Communications

Communications may include: • Telephone; • Voicemail; • Text; • Email; • Patient portal; • Other approved communication methods. Customers are responsible for keeping contact information current.
